Request for proxy access to GP online services
Use this form if you want someone you trust to have proxy access to your GP online services, including your clinical record. You can change your mind and ask us to remove access at any time. This access will remain in place until you tell us to remove it.
Important: Only agree if this is your choice. The person you choose may be able to see private or sensitive health information. If you feel pressured to give access, please speak to the practice privately.
We will need to see ID and supporting documentation for the person receiving proxy access before it is set up.
